Terms of Use
These Terms of Use constitute a legal agreement between you and MyHealth, LLC, doing business as ChartRequest (“ChartRequest,” “we,” “us,” or “our”). They govern your use of the ChartRequest application, including any content, functionality, and services available through it.
By accessing or using the application, you agree to these Terms of Use and our Privacy Policy. If you do not agree, you must not use the application.
Eligibility
The ChartRequest application is intended for individuals 13 years of age or older residing in the United States or its territories. By using the application, you confirm that you meet these eligibility requirements.
Changes to These Terms
ChartRequest may update these Terms at any time. Changes take effect immediately upon posting and apply to all continued use of the application. It is your responsibility to review the Terms regularly. Continued use of the application indicates acceptance of any revisions.
Access and Account Security
We may change or discontinue any aspect of the application without notice. ChartRequest is not liable if any part of the application becomes unavailable.
You are responsible for:
- Ensuring you have the necessary equipment and internet access.
- Making sure anyone using the application through your connection complies with these Terms.
To use certain features, you may need to provide accurate and complete registration information. By doing so, you consent to our use of that information in accordance with our Privacy Policy.
Prohibited Uses
You agree to use the application only for lawful purposes. You may not use the application in any way that:
- Violates any federal, state, or local law.
- Harms or attempts to harm others, including exposing minors to inappropriate content.
- Impersonates any person or misrepresents your affiliation with any entity.
- Disrupts, damages, or interferes with the functioning of the application.

The application may include links to third-party websites for your convenience. These sites are not controlled or endorsed by ChartRequest. We are not responsible for the content, policies, or practices of any third-party websites.
Privacy
Our Privacy Policy describes how we handle your information. By using the application, you agree to our collection and use of your information as described in the Privacy Policy.
Disclaimer of Warranties
The application and all services and content provided through it are offered ‘as is’ and ‘as available,’ without warranties of any kind. ChartRequest does not guarantee the accuracy, reliability, or availability of the application or its content.
Limitation of Liability
To the maximum extent permitted by law, ChartRequest and its affiliates will not be liable for any damages arising out of or related to your use of the application. This includes direct, indirect, incidental, consequential, or punitive damages, even if we were advised such damages were possible.
Governing Law
These Terms of Use are governed by the laws of the State of Georgia. Any legal action arising from your use of the application must be brought in the appropriate courts located in Georgia.
Limitation on Time to File Claims
Any claim related to these Terms of Use or the application must be filed within one year of the date the claim arises. After that time, the claim is permanently barred.
Waiver and Severability
If any provision of these Terms is found to be unenforceable, the remaining provisions will remain in full force. Failure to enforce any part of these Terms does not waive our right to enforce them later.
Entire Agreement
These Terms of Use and our Privacy Policy constitute the entire agreement between you and ChartRequest with respect to the application and supersede all prior agreements.
HIPAA and Privacy
ChartRequest is committed to protecting the privacy and security of your health information in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA), the Health Information Technology for Economic and Clinical Health (HITECH) Act, and applicable state privacy laws.
By using the ChartRequest application, you acknowledge that your protected health information (PHI) may be transmitted electronically through secure and encrypted channels to facilitate record requests authorized by you or your legal representative.
ChartRequest acts as a Business Associate when handling PHI on behalf of healthcare providers and complies with all applicable HIPAA regulations, including implementing appropriate administrative, physical, and technical safeguards to ensure confidentiality, integrity, and availability of PHI.
ChartRequest does not use your PHI for marketing purposes and does not sell or rent your health data. Any use or disclosure of your information is limited to what is necessary to fulfill the services you or your provider request.
You have the right to request access to your health records, request corrections, and file complaints about misuse or inappropriate disclosures of your PHI. If you have concerns about privacy or data security, please contact ChartRequest’s Privacy Office at
